Children's Long Term Support (CLTS)
Milwaukee County Disabilities Service Division contracts with Curative Care to provide Service Coordination for children and families currently participating in the Children’s Long Term Support (CLTS) waiver program and the Children’s Community Options Program (CCOP).
The Curative team provides treatment services and resources for children diagnosed with an Autism Spectrum Disorder and other disabilities or limiting conditions.
A professional Service Coordinator is assigned to each child served to help assess their individual needs and arrange for a variety of community services, including: respite care, home modifications, therapeutic services or adaptive aids. Other services may be arranged based on the child’s needs.
All services are based on each child’s assessed needs and goals, which are developed with input from the family and/or caregiver.
Curative Service Coordinators serve children up to age 22 who have a severe physical, developmental or emotional disability.
Children served must have a long-term care need and reside in Milwaukee County.

Curative staff work with children and families plus other organizations to maximize access to vital community resources. All services are based on each child's assessed needs and goals, which are developed with input from the family and/or caregiver.

Curative Care is committed to providing high quality, person-centered care and services. Our program outcomes are reviewed monthly to develop focused actions for continuous performance improvement.
Curative’s clients and their families work with our staff to assess what is important to the client. The team also establishes individual goals and staff-supported achievement plans.
Curative measures achievement of individual goals and asks for input from our clients and stakeholders to ensure the program is of value to persons served.

Hours and location
Visits with Curative Service Coordinators are typically conducted Monday through Friday from 7:30 a.m. to 4:30 p.m. Evening appointments may be available upon request. Based on the needs of the clients and families, services are primarily provided in the community.
