Quality Director -- Full-time position available at Curative - 92nd Street.

  • General Description: Reporting to the President & CEO of Curative Care Network, this position will play a key leadership role in developing quality indicators that measure the outcomes of the programs and services provided. Benchmarks and analyzes data to monitor performance of the organizational quality management systems and evaluates compliance to standards. Develops a quality dashboard and works with leaders to continuously improve performance. Remains current on quality standards, accrediting bodies and funding sources to fulfill quality strategic directions. Leads the CARF survey team.
  • Skills and Qualifications: Must have a Bachelor of Science in Nursing, with a Master’s degree preferred. Minimum of three (3) years’ experience in quality management and improvement required. Must have experience leading quality projects and team management. Excellent analytical skills and strong written and verbal presentation skills are required. Senior Financial Analyst -- Full-time position available at Curative - 92nd Street.

  • General Description: Provides leadership and supervision of day-to-day operations and assigned staff within area(s) of responsibility. Communicates organizational direction to department staff. Administers, interprets and ensures compliance of organizational policies and procedures within assigned area(s). May plan, develop and/or direct programs and/or services within assigned area(s). Provides analytical support to service lines across the organization by providing consistent and meaningful financial information, reporting and analysis. Monitors and assesses key performance metrics to refine processes and improve efficiencies. Identifies opportunities to improve operating performance. Prepares financial analysis and trending reports. Coordinates preparation of the monthly business review packages. Adult Day Services
 

Admissions and Outreach Coordinator -- Full-time position available at Curative - 92nd Street.

  • General Description: Functions as single point of accountability internally and externally for the Adult Day Services admissions process. As part of the Adult Day Services administration team, this person will be held accountable for increasing the number of referrals and admissions to all programs and improving the efficiency and effectiveness of the process. Coordinates and facilitates the admissions process for all Adult Day Services programs. Develops and implements strategies in order to reach recruitment goals. Maintains and builds relationships with referral sources, and seeks opportunities to raise awareness in the community of Curative Care Network and the services provided through Adult Day Services programs.
  • Skills and Qualifications: Requires a Bachelor’s Degree and a minimum of two years’ experience in recruitment, admissions and/or community relations. Experience working with individuals with disabilities also desirable. Must possess good interpersonal skills to communicate with persons served, families, co-workers and outside organizations. Must have excellent verbal/written communication skills and the ability to work with others in a team environment. Ability to work nights and weekends as needed. Program Coordinator -- Full-time position available at Curative - Menomonee Falls.

  • General Description: Provides assessment, program planning and service coordination for persons served. Plans, develops and implements activities in Adult Day Services for persons with developmental disabilities to assist them in attaining personal goals. Within the program, the Program Coordinator will work with the Program Specialists on planning activities, scheduling community outings and coordinating daily program activities. Will provide direct care and personal cares to assist in activities of daily living. May have other duties as assigned.
  • Skills and Qualifications: Requires a Bachelor's Degree in a related field or at least 2 years of experience as a Curative Care Network Program Specialist, with demonstrated ability to provide case management tasks according to Curative's standards. Children's Services
 

Family Service Coordinator -- Full-time position at Curative - 92nd Street.

  • General Description: Provides case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Conduct screenings, facilitate eligibility determination, organize and participate in evaluations and develop plans for care. Link families with community resources, teach families how to be effective advocates for their children, and assist families in arranging for services their children may need at the time of discharge. Demonstrate excellent documentation, organization and customer service skills.
  • Skills and Qualifications: Requires knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed. Bachelor’s Degree in a human service related field (human development, psychology, social services or similar areas) OR Three years or more experience as a Family Service Coordinator (or equivalent case management position) OR Successful completion of the Human Services Associate Degree internship program through MATC and Curative Care Network. Case Manager, Children's Case Management -- Full-time positions at Curative - 92nd Street.

  • General Description: As a member of the Children’s Care Management Team, the Case Manager will be responsible for locating, managing, coordinating and monitoring all proposed waiver services, other services and informal community supports needed by the children and their families. This entails going into the clients' homes in their community.
  • Skills and Qualifications: The minimum requirements for this position are: Be a Social worker certified in Wisconsin or a Bachelor’s degree in a social services area (e.g. Human Services, Social Work, Psychology, Nursing, Rehabilitation, or related field).   • General Description: A member of the Interdisciplinary Team (IDT) that is responsible for the coordination of case management services provided to members, families, providers and other stakeholders throughout Milwaukee County. This entails going into the clients’ homes in their community.
  • Skills and Qualifications: The minimum requirements for this position are: Be a Social Worker certified in Wisconsin or have a four-year Bachelor’s degree in a social services area (e.g. social work, rehabilitation, psychology, etc.) as well as at least one year of formal (non-family setting) experience working with disabilities or frail elders, or have a four-year Bachelor’s degree in any other area with a minimum of three years of formal (non-family setting) experience working with persons with disabilities or frail elders. In addition, the Case Manager must also have knowledge of community resources available to meet the needs of the members served and a thorough understanding of the range and type of long term care options available in the community. Must have a valid Wisconsin driver’s license, reliable transportation and automobile experience.
