  • General Description: Implement daily activity plans for our clients with disabilities, including crafts, games, and community outings. Provide input regarding goal achievement of clients. Supervision for clients during unstructured time. Assist clients in meeting their personal care needs. May have other duties as assigned.
  • Skills and Qualifications: High School diploma or equivalent. Ability to work with others in a team environment. Preferred skills: Experience supporting adults with Alzheimer's, Dementia and memory loss helpful. Case Management Services

Case Manager, Adults/Seniors -- Full-time position at Curative - 92nd Street.

  • General Description: A member of the Interdisciplinary Team (IDT) that is responsible for the coordination of case management services provided to members, families, providers and other stakeholders throughout Milwaukee County. This entails going into the clients’ homes in the community.
  • Skills and Qualifications: Be a Social Worker certified in Wisconsin, or have a four-year Bachelor’s Degree in a social services area (e.g. Social Work, Rehabilitation, Psychology or related field), as well as at least one year of formal (non-family setting) experience working with persons disabilities or frail elders OR have a four-year Bachelor’s Degree in any other area with a minimum of three years of formal (non-family setting) experience working with persons with disabilities or frail elders. Requires a valid Wisconsin driver’s license, reliable transportation and automobile insurance. Local travel, at a minimum southeastern Wisconsin but usually within Milwaukee County.   • General Description: Reporting to the Chief Executive Officer, the Director oversees the coordination and administration of all aspects of the Case Management division, including planning, organizing, leading and coordinating staffing, quality and program evaluation as well as contractual and funding source compliance. The Director will ensure that the delivery of the overall program and its activities are in accordance with the mission and vision of Curative.   • Skills and Qualifications: Requires a Bachlor's’s Degree with an emphasis in Rehabilitation, Healthcare Administration or Business Administration. Master's Degree preferred. Must be a Licensed Professional in Physical Therapy, Occupational Therapy, Speech Language Pathology or Early Childhood Education with 10 years of progressive leadership roles in the rehabilitation or healthcare field. Candidate must be an accountable leader with excellent interpersonal skills and the ability to relate and communicate with clients, direct line staff, managers, guardians/caregivers, funding sources and stakeholders.
