Adult Day Services
 

Program Coordinator-- Full-time position at Curative - Individualized Day.

  • General Description: Acts as primary liaison between Curative and the clients' guardian and stakeholders. Provides assessment, program planning and service coordination for persons served. Plans, develops and implements activities for persons with developmental disabilities to assist them in attaining personal goals. Works with the Program Specialists on planning activities, scheduling community outings and coordinating daily program activities. Will provide direct care and personal cares to assist in activities of daily living. May have other duties as assigned.
  • Skills and Qualifications: Requires a Bachelor’s Degree in a related field or at least 2 years of experience as a Curative Care Network Program Specialist, with demonstrated ability to provide case management tasks according to Curative’s standards. Must have at least one year experience working with developmentally disabled persons. Must possess good interpersonal skills to communicate with persons served, families, co-workers and outside organizations. Case Management Services
 

Case Manager, Adults -- Full-time position at Curative - 92nd Street.

  • General Description: A member of the Interdisciplinary Team (IDT) that is responsible for the coordination of case management services provided to members, families, providers and other stakeholders throughout Milwaukee County. This entails going into the clients’ homes in their community.
  • Skills and Qualifications: Must be a Social Worker certified in Wisconsin, or have a four-year Bachelor’s degree in a social services area (e.g. Social Work, Rehabilitation, Psychology or related field), as well as at least one year of formal (non-family setting) experience working with persons disabilities or frail elders, OR Have a four-year Bachelor’s degree in any other area with a minimum of three years of formal (non-family setting) experience working with persons with disabilities or frail elders. Requires a valid Wisconsin driver’s license, reliable transportation and automobile insurance. Local travel, at a minimum southeastern Wisconsin but usually within Milwaukee County, is required. Registered Nurse-- Full-time position at Curative - 92nd Street.

  • General Description: A member of the Interdisciplinary Team (IDT) that is responsible for monitoring the health and safety of a caseload of members. Regular contact with members, families, providers and other stakeholders in the community at home or other locations as indicated.
  • Skills and Qualifications: Requires completion of an accredited professional training program for Registered Nurses - minimum of Associates Degree; Bachelor’s Degree preferred. Must hold licensure to practice nursing in the State of Wisconsin and have practiced clinical skills for at least one year. Must possess basic computer skills and word processing competencies. Family Service Coordinator -- Full-time position at Curative - 92nd Street.

  • General Description: Provides case management/service coordination for children ages zero to three who evidence a developmental delay or other atypical development. Coordinator conducts screenings, facilitates eligibility determination, organizes and participates in evaluations and develops plans for care. Coordinator also links families with community resources, teaches families how to be effective advocates for their children, and assists families in arranging for services their children may need at the time of discharge. Demonstrates excellent documentation, organization and customer service skills.
  • Skills and Qualifications: Requires knowledge of the local service delivery system, the target group’s needs, the need for integrated services, and the resources available or needing to be developed. Requires a Bachelor’s Degree in a human service related field (human development, psychology, social services or similar areas) OR three years or more experience as a Family Service Coordinator (or equivalent case management position) OR successful completion of the Human Services Associate Degree internship program through MATC and Curative Care Network.   • General Description: Plans, organizes and implements job training, job coaching and production activities for persons served working in the Pre-Vocational Training Program. Provides daily assistance to persons served in the Pre-Vocational Training Program.
