  • General Description: This is an entry level position, providing one-on-one supervision and assistance to assigned client during structured and unstructured time. Implement daily activity plans and goal achievement for assigned client. Provides assistance in meeting their personal care needs. May have other duties as assigned.

Case Manager, Children's Case Management -- Full-time positions at Curative - 92nd Street.

  • General Description: As a member of the Children’s Care Management Team, the Case Manager will be responsible for locating, managing, coordinating and monitoring all proposed waiver services, other services and informal community supports needed by the children and their families. This entails going into the clients' homes in their community.
  • Skills and Qualifications: The minimum requirements for this position are: Be a Social worker certified in Wisconsin or a Bachelor’s degree in a social services area (e.g. Human Services, Social Work, Psychology, Nursing, Rehabilitation, or related field). Minimum of two (2) years’ experience coordinating client services in a field of developmental disabilities or a related field. Bilingual in Spanish preferred. Must have knowledge of Wisconsin’s long-term support funding sources, as well as Medicaid Waiver programs. Excellent communication skills required. Case Manager, Family Care -- Full-time positions at Curative - 92nd Street.

  • General Description: A member of the Interdisciplinary Team (IDT) that is responsible for the coordination of case management services provided to members, families, providers and other stakeholders throughout Milwaukee County. This entails going into the clients’ homes in their community.
  • Skills and Qualifications: The minimum requirements for this position are: Be a Social Worker certified in Wisconsin or have a four-year Bachelor’s degree in a social services area (e.g. social work, rehabilitation, psychology, etc.) as well as at least one year of formal (non-family setting) experience working with disabilities or frail elders, or have a four-year Bachelor’s degree in any other area with a minimum of three years of formal (non-family setting) experience working with persons with disabilities or frail elders. In addition, the Case Manager must also have knowledge of community resources available to meet the needs of the members served and a thorough understanding of the range and type of long term care options available in the community. Must have a valid Wisconsin driver’s license, reliable transportation and automobile experience. Supervisor, Children's Case Management -- Full-time position at Curative - 92nd Street.

  • General Description: Provides supervision of the day-to-day operations, communicate organizational direction to departmental staff, administer, interpret, implement and ensure compliance of organizational policies and procedures within the Children’s Case Management department. Monitor service utilization, review monthly case notes and provide feedback to Case Managers as well as Milwaukee County as requested. Provide orientation and training in Case Management processes to all new case managers, promoting individualized care and natural support.
  • Skills and Qualifications: The minimum requirements for this position are: Be a Social worker certified in Wisconsin or: Bachelor’s degree in a social services area (e.g. Human Services, Social Work, Psychology, Rehabilitation, or a related field); minimum of two (2) years’ experience coordinating client services in a field of developmental disabilities or a related field; minimum of two (2) years’ supervisory experience and/or experience in a leadership role in a disabilities program and working in a managed care environment; excellent communication skills; basic computer skills and word processing competencies.
