Director of Adult Day and Senior Services -- Full-time position available.

  • General Description: Reporting to the Chief Executive Officer, the Director of Adult Day and Senior Services oversees the coordination and administration of all aspects of the Adult Day and Senior Services programs including planning, organizing, staffing, leading and coordinating program activities. The Director will ensure that the delivery of the overall programs and its activities are in accordance with the mission and vision of Curative. The Director is responsible for the quality of care and programming provided to our clients that will assist our clients achieve their individual goals. The Director will develop new initiatives to support the strategic direction of the programs along with implementing long-term goals and objectives to achieve the successful outcomes.

Case Management Services - Adult

Case Manager, Adult -- Full-time positions available at Curative - 92nd Street.

  • General Description: As a member of the Interdisciplinary Team (IDT), the Case Manager works with a Registered Nurse to provide case coordination to adults ages 18 and up who have long-term disabilities related to development, aging, and/or physical disabilities. Responsible for identifying each members’ long-term goals, natural supports and paid services, which are then funded through My Choice Family Care. Responsible for maintaining monthly contact with each member, completing a psycho-social  assessment at least every six (6)months, assisting them with maintaining their T-19 eligibility and with connecting members to community based resources. The Case Manager will see the member at their home, group home, nursing home, day program, work environment and within community schools.
  • Skills and Qualifications: Must be a Social Worker certified in Wisconsin OR have a Bachelor's Degree in a social services area (Social Work, Rehabilitation, Psychology, etc) as well as at least one (1) year of formal (non-family setting) experience working with persons with physical and developmental disabilities and/or frail elders OR have a Bachelor’s Degree in any other area with a minimum of three (3) years of formal (non-family setting) experience working with persons with disabilities or frail elders. Manages a team of Job Coaches and Placement Specialists to ensure the team is conducting vocational job exploration, implementing job development strategies, supporting interviewing techniques and resume development, while matching clients to competitive jobs with committed community employers.
  • Essential Duties and Responsibilities: Ensures effective program operations and compliance with funder requirements, goals, and expectations. Also ensures the Employment Services team is building relationships with committed community employers to match clients to competitive jobs. Must have knowledge about workforce trends, such as job carving, employment first, and talent management. Is knowledgeable in the area of WIOA, CMS, HCBS, DOL, DWD, and long term care. Manager represents Curative Care Network in the rehabilitation community and develops relationships with professional associations, workforce development groups and professional committees on a local and state level. Works as a team member with other Curative departments to develop integrated programming. Provides leadership and programmatic strategy to meet and/or exceed targets. Responsible for the overall fiscal management and operations, including working with the Finance team to ensure expense management and revenue cycle management. Must have the ability to develop new revenue streams. Works collaboratively with the Marketing and Communications team to brand the Employment Center as a business to business resource in the community.
