Case Management Services

Curative Care's Case Management Services provide individualized support and supervision to children and adults with physical and developmental disabilities, as well as to frail elders. Staff members work in partnership with each individual and various agencies to maximize access to community resources.

Pediatric Case Management

Milwaukee County Disabilities Service Division contracts with Curative Care to provide Service Coordination for children and families currently participating in the Children’s Long Term Support waiver program.

The Curative Pediatric Case Management Team provides treatment services and resources for children diagnosed with an Autism Spectrum Disorder and other disabilities or limiting conditions. 

A professional Service Coordinator is assigned to each child served to help assess their individual needs and arrange for a variety of community services, including: respite care, home modifications, therapeutic services or adaptive aids. Other services may be arranged based on the child’s needs.

All services are based on each child’s assessed needs and goals, which are developed with input from the family and/or caregiver.

Curative Service Coordinators serve children up to age 22 who have a severe physical, developmental or emotional disability.

Children served must have a long-term care need and reside in Milwaukee County.

Resource Centers in Milwaukee County accept applications for Children’s Long Term Support (CLTS) services. Call the Milwaukee County Disability Services Division at 414-289-6799.

Adult Case Management

My Choice Family Care contracts with Curative to provide Case Management Services for adults with physical and/or developmental disabilities as well as frail elders.

Professional Case Managers and Registered Nurses assist persons in obtaining a variety of services and supports.

All services are based on each client’s assessed needs and goals, which are developed with input from the client, family and/or caregiver. 

Services that are coordinated may include: adaptive aids, adult day care, residential care, communication aids, consumer education and training, counseling and therapeutic counseling, financial management services, home delivered meals, home modifications, housing counseling, personal emergency response systems, relocation services, respite care, specialized medical equipment and supplies, supportive home care and specialized transportation. Other services may be arranged based on individual needs.

Persons served must have a long-term care need and receive funding through Milwaukee County.

Resource Centers in Milwaukee County accept applications for My Choice Family Care services. For those aged 18 - 59, call the Disability Resource Center at 414-289-6660. For those over the age of 59, call the Aging Resource Center at 414-289-6874.

A partnership dedicated to service and support

Curative staff work with persons served and other organizations to maximize access to vital community resources. All services are based on each individual’s assessed needs and goals, which are developed with input from the family and/or caregiver.

Persons served 

  • Service Coordinators serve children up to age 18 with severe physical, developmental or emotional disabilities. Children served must have a long-term care need.
  • Case Managers serve adults aged 18 and older with physical and/or developmental disabilities as well as frail elders. Persons served must have a long-term care need.

Core Team Members

  • Curative Service Coordinators provide information and arrange services based on the long-term support needs of children served.
  • Curative provides an interdisciplinary team including a Case Manager and Registered Nurse to provide information and arrange long-term support services for adults.

Curative Care is committed to providing high quality, person-centered care and services. Our program outcomes are reviewed monthly to develop focused actions for continuous performance improvement.

Achieving Outcomes

Curative’s clients and their families work with our staff to assess what is important to the client. The team also establishes individual goals and staff-supported achievement plans.

Curative measures achievement of individual goals and asks for input from our clients and stakeholders to ensure the program is of value to persons served.

Hours and location

Case Management Services visits are conducted Monday through Friday from 7:30 a.m. to 4:30 p.m. Services are primarily provided in the community based on the needs of clients.
