disabilities in Milwaukee County.
These
programs include:

Family Care case management program for older adults.

Supported Living program for persons with disabilities who wish to
live independently in their home.

Personal Planning program for persons with disabilities who are
on a waiting list for funded-services.

Case
management for adults with physical disabilities.
Case
managers work in partnership with persons served and various agencies to
coordinate services received.
