Curative Care Network Case Managers work with adults enrolled in the Milwaukee County Department of Family Care program to:

- Get the community services and supports they need

- When they need them

- And for the best value

The Milwaukee County Department of Family Care program provides managed long-term care services to frail elders aged 60 and older, as well as to adults aged 18-59 who have physical and/or developmental disabilities. Services foster independence in the home and community.

Curative's Family Care team assists individuals and their families in locating, managing, coordinating and monitoring all proposed services and informal community supports, including:

- Development of an Individual Service Plan

- Completion of functional screening

- Arrangement for provision of services

- Service coordination

- Assessment

- Crisis intervention

The Family Care Team includes the program member, a Curative Registered Nurse, Case Manager and Family Care Program Manager.
