Curative's Physical Disabilities Case Management Program - through the Milwaukee County Adult Services Division - provides case management and support services to persons between the ages of 18 and 59 who have been diagnosed with a physical disability. Services foster independence in the home and community.

Through the COP Waiver Program, Curative's Physical Disabilities Case Management team assists individuals and their families in locating, managing, coordinating and monitoring all proposed services and informal community supports. Services under the COP Waiver Program include:

- Development of an Individual Service Plan

- Completion of functional screening

- Arrangement for provision of services

- Service Coordination

- Assessment

- Crisis intervention

The Case Management Team includes the program member, a Curative Case Manager assigned to provide assistance and a Curative Case Management Manager.
