Curative's Family Care Program - through the Milwaukee County Department on Aging - provides case management and support services to Milwaukee County residents over age 60. Services foster independence in the home and community.

Curative's Family Care team assists individuals and their families in locating, managing, coordinating and monitoring all proposed services and informal community supports. Services include:

- Development of an Individual Service Plan

- Completion of functional screening

- Arrangement for provision of services

- Service Coordination

- Assessment

- Crisis intervention

The Family Care Team includes the program member, a Curative Registered Nurse, Case Manager and Family Care Program Manager.
