Curative Care Network Case Managers work with Milwaukee County adults who have physical disabilities and/or developmental disabilities, as well as older adults to:

- Get the health care and other community services they need

- When they need them

- And for the best value

- Case Management for adults with physical disabilities

- Case Management for adults with developmental disabilities

- The Family Care case management program for older adults

- A Personal Planning program for persons with disabilities who are on a wait list for funded services.
